| NOT A SUBSTITUTE FOR MEDICAL EXAMINATION |
I understand that massage should not be construed as the substitute for medical examination or treatment and that I should see a physician, chiropractor or other qualified medical specialist for any mental or physical ailment I am aware of. I affirm that I will tell my provider all my known medical conditions BEFORE MY MASSAGE APPOINTMENT. I agree to keep the provider updated as to any changes in my medical profile, and understand that there shall be no liability on the practitioner's part should I forget to do so. |
| THERAPEUTIC PURPOSES ONLY |
The providers listed on this website perform massages for therapeutic purposes only. It is understood that any inappropriate behavior such as excessive drinking, drug use, illicit or sexually suggestive remarks, or advances made by me will result in immediate termination of the session, and I will be liable for payment for the "full" scheduled appointment. After the dispute is evaluated by Go Massage, and Go Massage determines that the client was at fault, the provider has the right to use Go Massage to process the client's credit card. |
| WEBSITE MEMBERSHIP - LISTING SERVICE ONLY |
Go Massage is an online directory for people seeking massage therapy by certified professionals at a massage facility or at their home, office, hotel, or special event. The client agrees to pay the appropriate membership fee in order to be a member of this site. If a client is on a monthly program (autorenew), he/she can cancel at anytime. Simply give Go Massage a call. Client will be billed up to the end of that current billing period. If client has a 6 month or 1 year membership, he/she will not receive any funds back if the membership is cancelled. If a client makes an appointment request through the website, he/she understands that there may not be a suitable provider in his/her area. Client also understands that if a message is left for a provider that a provider may not return the call or email. If client is under the age of 16, an adult must be present somewhere in the same location at all times during the massage. Client agrees that posting any bad language or offensive remarks in his/her reviews of providers or Go Massage in general will result in the termination of his/her membership. Any client feedback or reviews of therapists, etc. that are submitted to Go Massage by clients or potential clients via web forms or email may be used in publicity material such as website promotions, brochures, newsletters, or other publications. Using any specials more than once per client is against Go Massage Policies and may result in the termination of a client's membership. All financial transactions regarding massage appointments are between the client and the provider. However, Go Massage is used as a credit card processing service. |
| CLIENT DISPUTES WITH PROVIDERS |
Any disputes between client and provider have nothing to do with Go Massage. However, client is free to post a negative review of that provider on the Go Massage website if he or she is unhappy with the service provided. Provider can also post a rebuttle to that review. If, for some reason, the provider does not show up to said appt. or is extremely late, Go Massage cannot take any responsibility for that and cannot offer any refunds for its membership fees. |
| SCREENING PROCESS OF PROVIDERS |
There are some things the client can do to assist in the selecting a competent, professional individual for services: - Be specific about the massage flavor (modality) that he/she desires. |
| APPOINTMENTS - PAYMENT TYPE |
Clients can pay for appointments by cash or credit card, however all concierge mobile appts. must be paid by credit card. Event appointments can be paid by credit card or check. |
| APPOINTMENTS - GRATUITY |
Rates for providers using Go Massage do not include gratuity. |
| APPOINTMENTS - REFUNDS |
Go Massage does not offer any refunds however you can request a refund from the individual provider. |
| APPOINTMENTS - CANCELLATION |
Clients using Go Massage have a 12 hour per provider cancellation policy. That means if the client books 3 providers, he/she needs to give them each 12 hours notice for cancellation. If providers do not receive a notice, then the client can be charged the full amount of the appointment if the provider(s) reports the incident to Go Massage. If the client books the appointment from 12 hours to 1 hour before the scheduled appointment, there is no cancellation, and he/she will be charged the full amount of the appointment if the provider reports the incident to Go Massage. For Event Appointments (Spa Parties, Corporate Massage, etc.), Go Massage requires a 24 hour notice per provider for cancellation. Failure to give this notice will result in being charged the full amount of the appointment. |
| APPOINTMENTS - RESCHEDULING |
For regular appointments (see definition in table above), the rescheduling of appointments is between the provider and the client. However, if the client does not give 12 hours notice prior to rescheduling, the provider does have the right (if he, she chooses) to charge the client for the full appointment. For concierge appointments, there is a one time $15.00 fee to the client if the client reschedules the appointment. For event appointments, the fee will vary depending on the circumstance. |
| APPOINTMENTS - LATENESS |
CLIENTS: Clients have a 15 minute grace period for their massage. If he/she arrives at said location within 15 minutes of the appointed time, there will be no additional charges. Anything beyond 15 minutes will be taken off the time of the massage.

| APPOINTMENTS - NO SHOWS |
CLIENTS: If the massage provider shows up at the location, and the client is not there within 15 minutes of the specified time, then the massage does not have to be performed, and the massage provider has the right to charge the client in FULL using the credit card on file with Go Massage. |
| APPOINTMENTS - MASSAGE DURATION / RATE |
Client agrees to pay the posted rate of the provider which was listed on the website at the time the appointment was booked. A 60 minute massage time includes dressing and undressing but it does not include travel time, setting up the table, or putting away the table. The actual hands on time will be approximately 55 minutes. For mobile appointments, clients should make sure they have a room that is cleared for the table before the provider arrives. |
| APPOINTMENTS - PARKING POLICY |
If there are any parking charges (usually at a hotel or office), then that amount must be re-imbursed
to the provider. |
| APPOINTMENTS - TRAVEL POLICY |
MOBILE APPTS. ONLY - If there are no providers available within a 25 mile radius of your location, but the client is still interested in booking a massage, an extra $15.00 travel fee to the provider may apply. This is something to be discussed with the provider ahead of time. |
| APPOINTMENTS - SPECIALIZED MODALITY / AROMATHERAPY POLICY |
MOBILE APPTS. ONLY - For deluxe massages (Shiatsu, Pregnancy, Lomilomi, Reiki, Reflexology, Manual Lymph Drainage), client agrees to pay an additional $10/hr. more than the standard rate. For Stone Massages, I agree to pay $20/hr. more. If provider brings aromatherapy or a candle, he/she can charge an extra $10.00/person. |
| APPOINTMENTS - PROVIDER / CLIENT MISCONDUCT |

If reports come back from client that there was provider misconduct, Go Massage has the right at its sole discretion to take provider’s profile off the website. |
| FINAL AGREEMENT |
By using this site, myself and my party are acknowledging that we are at least 18 years of age and understand the risks described above. Go Massage reserves the right, at our sole discretion, to change, modify or otherwise alter these terms and conditions at any time. Such modifications shall become effective immediately upon the posting thereof. I understand it is my responsibility to keep up to date on them. If a disagree with any new terms, I can cancel at any time, and if I don't obide by them, Go Massage has the right to suspend my membership. |
